UPDATE DRAFT-African Diaspora Medical Project Application
Marge created the attached draft for the application to the African Diaspora Medical Project. There's been so much going on that we still have not mailed this out, but we also wanted to wait and see if we would be successful in securing the hospital beds through Friends of Kenya first-which we were, so, now the draft application needs to be updated.
Bramuel and I have been discussing this application over the last several weeks. Bramuel has created a list with cost estimates of the equipment most needed at Arrow-Web Hospital. I also asked him to sit with the Chief Medical Doctor and go through each piece of equipment and answer two questions:
1. Why is it needed?
2. What happens now without it? What does it mean to someone who may need it, but can't access it becuase Arrow-Web Hospital does not have the equipment.
For this task, I would like a volunteer to take another look at this application and consider the attached information. Help us come up with a final draft that we can submit for consideration. Please let me know if you are willing to take on this task.

Have only had a quick read of everything but one question occurs to me....
As well as looking at the benefits of the equipment, has the hospital also looked at whether there will be any additional costs involved and whether they can meet them?
For example what training they would need to provide to staff, whether they would need to employ additional people e.g. to operate xray and ultrasound, what ongoing running costs there will be?

Yes when we have get xray machine we will need to add one staff for that purpose only to operate and manage the x-ray machine
Ultra sound is the some as X-ray this is part of our long term solutions and sustainability to the hospital.

Just to clarify - do you mean that you will need one extra member of staff and they can run both machines, or that you will need two more people? And assuming you are successful in getting funding to buy the machines, you will be able to afford to pay another member of staff?

I have started to try and modify the application to bring it up to date. Have made a few changes, which are marked as revisions, also asked a few questions which are in the doc as comments.
I have to stress that I am NOT an expert at this!
Have not got all the way there, but thought I would post in case anyone else is working on it and we can combine efforts.

yes we will need to add two more staff to operate the ultra sound machine and x-ray machine
and am sure that the hospital will manage to pay this two staff since the machine will not only safe the hospital patients but we will be getting some referral patients for x-ray or ultra machine and those who sent from other hospitals they pay.this another long term solutions to the hospital.

I have updated the doc to include details of the equipment and it is attached. I have pulled the info out of the advantages/disadvantages doc and reworded a little - hopefully it is still accurate.
Changes are marked as revisions, and there are also some questions/comments that are marked as comments.
I am sure it is not totally complete, so would welcome input from others.

I am feeling very hesitant about submitting this application for all of the equipment needed rather than just a few pieces of it.
Mary, I think your additions are good and it will help make this easily adaptable for submission. I do think you made an excellent point about what happens if we DO get help with getting some of this equipment in terms of maintenance costs, costs to operate it and whether or not the hospital income can realistically support those costs at the present time. I am really concerned about making a strong case in our application for how the equipment requested will result in long-term sustainability.
With the $1500 USD in medications that we need to stock in the pharmacy right now, I think that has to take priority for the time being in order to keep the accreditation and assistance from NHIF. Once we have a few months of providing services to their card-carrying members I think we're going to know more about what this means in terms of income to the hospital. We should be able to see how much this defrays some of the expenses so that money can be used to support other things.
We have time, so we'll continue working on it and make adjustments as we need to.

I agree that it makes sense to concentrate on the medication first as that is pretty central to the hospital's survival. Have posted a couple of comments about that under the other task. One option may be to put in an application for e.g. 6 months supply with clear information on future sustainablilty. Or for that equipment that does not have high running and maintenance costs. Its possible that, if they decide to support the hospital, they would then sonsider funding for other items in the future.
Sorry, not quite sure to post this (it doesn't really live under this task) - came across a list of organisatoins that send used equipment to hospitals in developing countries. Am too tired to look at this now, but may be an option for some things, if they pay shipping. http://www.operationgivingback.facs.org/content78.html

Bramuel and I have been emailing back and forth additions/revisions to the application. I think it's done-just waiting on his final approval of some last minute edits. I just noticed too that it's about a page longer than they want, but I'm not sure exactly where we could cut down on the information we included. I hope to get this submitted in the next day or two so we can close this task and move on.
